Laboratory Accreditation Inspection Checklists

Inspection Checklist Questionnaires

In an effort to ensure quality patient care in the Province of Saskatchewan, the Laboratory Quality Assurance Program conducts accreditation visits for medical laboratories.

As one other component of a quality system, the inspection or audit process is detailed in the inspection checklist documents. These documents identify the expected standards of practice for accreditation. There is a checklist for each laboratory discipline. These questionnaires are readily available to all labs.

When an audit has been designated for your facility, it is your responsibility to complete the checklist document for all tests that you do and submit to the LabQA Program within the defined timeframe. An audit team will arrange an accreditation visit and will compare the checklist responses to the actual practice within your laboratory.

These checklists identify areas for improvement or education and serve as the foundation for a peer review process. The value of an audit cannot be understated. It provides a consistent format for testing and ensures the quality of the result.

Laboratories are encouraged to actively use the documents in preparation for the official audit.
